This is all the information you will need on your invoice: Customer name, address and phone number, date promised, the deposit received and the balance due.

This order form is not for your customer but for your own information when you accept custom orders.

This is not in the form you will use but is an indication of all the items you will need to place on your form. I personally like an 8 1/2 X 11 order form set up this way for myself with this information on it which I can keep in my binder until the order is filled and delivered. It can then be filed in case you get another order for the same item. You may find another arrangement more suitable for you. As long as all the information is included set up yours as you like.

The order form is for your eyes only so make any notes to yourself that will help you complete the project efficiently.

Date to be delivered: This should be placed in the upper right hand side of the page so it can be seen easily. Ask for a best time to deliver or for customer pick up or date to ship.
Sender's name if this is a gift and fill in the information accordingly: If a corporate gift this should be the company name, followed by the person ordering.

Telephone number
recipient's name:
Delivery address: If the delivery area is not on a map and unfamiliar to you it is a good idea to get directions either from the sender when a personal gift or call the recipient for directions. If you are shipping the product be sure to include any tracking numbers here.

Occasion: If for a special occasion or special instructions like personalization.
Name of craft item:
Price are the tax & delivery included?
Special requirements: Custom colors etc.
Gift card to read as follows: if a gift.
Method of payment: check, money order, cash, credit card or on account. On personalized custom orders a non refundable deposit should be required.
Is there anything else I can do for you? Be sure to ask.
Don't forget to thank them for the order.

Special Note: If you are asked to prepare this item as a gift for an occasion be sure you know what the occasion is, if from a different culture ask for help. You do not want to take the chance of an inappropriate for the occasion gift. Write out your questions. Keep your answers -- you may need the same information again.
